This gorgeous long sleeve gothic blouse top is by Bares by Fashion-X clothing.  It is made from black stretch velvet and has a ribbon lace-up down the centre panel.  The neckline is trimmed with lace.  The neckline is the same shape as the 3030 top shown in the second pic.

The sleeves are black velvet to below the elbow, and then flared black net below the elbow.
